The death is registered by The Civil Registry Office (USC) for the place where the death occurred (not the place of residence of the deceased). On the basis of the medical certificate of death, the registrar draws up the death certificate and issues copies.
Who can register the death and collect the certificate?
- Immediate family - spouse, children, parents, siblings, grandchildren.
- Authorised representative appointed by the family.
- Funeral home - in practice it very often obtains the death certificate on the family's behalf.
How long does it take and how much does it cost?
The death certificate is usually drawn up immediately or within one day working day. The first abridged copy is issued free of charge. Further copies are subject to a stamp duty (a few dozen zloty per copy). It is therefore worth requesting a larger number of copies straight away.
How many copies of the death certificate should you have?
In practice it helps to have several copies - a separate copy may be needed for: the cemetery, ZUS, the bank, the employer and inheritance matters. A missing copy can hold up the next matter, so it is better to have spares.
